Step 1: Understand Process

Understand how the application process works and the Malka Ella Fertility Fund terms and conditions for general funding.

Treatments Funded

Artificial Insemination (AI) - the Fund covers up to 6 attempts per 12 month period.
In Vitro Fertilisation (IVF), Gamete Intrafallopian Transfer (GIFT) & Intracytoplasmic Sperm Injection (ICSI) - the Fund covers up to 2 attempts per 12 month period.
Adoption, Egg Donation, & Surrogacy - each case is individually considered.
Pre-Genetic Screening (PGS) & Pre-Genetic Diagnosis (PGD) - each case is individually considered.

Funding Criteria

To qualify for assistance, the following criteria must be met:

The couple must be halachically married by a Rabbi in an Orthodox Jewish ceremony.
The couple must complete a confidential financial assessment.
The couple must meet with the Malka Ella counsellor prior to the application being processed.
The Malka Ella Rabbi is required to sign off the procedure.
All procedures require supervision and must be performed in a halachically acceptable manner.

Terms and Conditions

Conditions of receipt of funds from the Malka Ella Fertility Fund are:

The woman needs to attend the mikveh before the required procedure.
A Malka Ella appointed mashgicha must be present in the laboratory at all times when procedures are being performed.
Sufficient notice must be provided to SHIFRA (Malka Ella division in charge of supervision), to ensure that a mashgicha is present at all procedures. The Fund will not pay for any procedures that are not supervised.
No procedures may be done on Shabbat or Yom Tov.
Funding and financial decisions are at the sole discretion of the coordinators of the Fund.
Financial assistance is only considered for expenses directly related to fertility procedures and not covered by the couple's medical aid.
All payments are made directly to the doctor or hospital, and will only be actioned once a written quote or invoice is provided.
Permission is given to the Fund to make contact with the treating doctor to discuss individual cases.
The couple must inform the Fund once a result is known.
Once a positive pregnancy result has been obtained, no further expenses will be paid.

All information provided to the Fund is handled in the strictest confidence.

Step 2: Collect Documents

To process your application, Malka Ella requires several documents to me made available in a digital format along with your submission. Please make sure the following documents are available before filling in the application form.

Rabbi's Consent

The Rabbi's Consent Form must be completed by Rabbi Gidon Fox and then uploaded with your application. Rabbi Fox can be contacted on [email protected]

Financial Documentation

You need to do the following:

Download Financial spreadsheet
Complete Financial spreadsheet with all the relevant information
Upload Financial spreadsheet along with digital copies of all supporting financial documentation required.

Please ensure documents are labelled with your surname and description of the document. For example if your surname is Gold your financial form should be labelled "Gold financial form.xlsx"
